If you would like to see a statement of your account and make an online payment with your credit card, complete the form and click on the "Submit" button. Upon verifying your information, a PDF of your account statement will be emailed to you within 72 hours. Within the email, a link will be available to make a payment by credit card (Visa, MasterCard, American Express, Discover) on our secure server. If the information you submit does not match our records, you will not receive a statement.
